Account page and nationality

The account page was reworked to make the details clearer:
- sign out button was moved up
- player rank is now displayed in more visible way
- Premium Token icon was added to make it clearer where the information about the amount of tokens available is located
- global ranking and national ranking position are now more visible in more graphic way
- current cases were added icons so it would be easier to navigate from this place

Also, an option was added for the users, who did not declare their nationality, so they could assign their country later. Just a quick reminder - you need to have the country assigned so you can be placed on the ranking for your country. It sounds obvious, but it is better to put it in writing, just in case.
